Insurance is
provided by American International Group/American International
Companies (not by YMCA).
You have
received an insurance card and information booklet with your DS-2019
form. If you need to go to a hospital, clinic, or doctor:

Contact AIG/Coalition America (800-878-7896) for preferred
healthcare providers in your location. You do not have to use
these healthcare providers, but doing so will decrease your
out-of-pocket costs.

Take the
insurance claim form with you.Click

Please insert the new claim
form, attached.

Take your
insurance card with
you. You originally received this with your DS-2019 package.
Most clinics and hospitals will not treat you until you can
prove that you have insurance.

You will need to pay $100.00
the first
time a doctor treats you for each separate accident or illness.
This is called the “deductible” and is not paid by the insurance
company.

Give the
claim form to the doctor's office; tell them you have accident
and sickness insurance underwritten by
American International Group
(AIG).

Show the
Doctor's office the insurance group # located on the back of the
insurance identification card. If the office doesn't recognize
the insurance, ask them to call:
1- 800-551-0824 or
1-302-661-4176.

Sign the
medical claim form and leave it with the doctor, clinic, or
hospital before you leave.
If you forget to leave the claim
form with the hospital or doctor's office, you will be
responsible for the cost of the medical treatment beyond the
deductible.

YMCA must be notified of any emergency incident within 24 hours,
either by the Trainee or the Host Site.
If you received health services and
did not give the medical office your insurance claim form, or the
office refused to bill the insurance company, you will have to pay
the expenses yourself and get a reimbursement by sending the claim
form along with the original medical bill to AIG.
Keep photocopies of all documents for yourself.
